The LVN/LPN provides professional nursing care to patients within an assigned unit, clinic, or region.Job Functions

  • The LPN/LVN assesses patient health problems and needs.
  • Takes vital signs, administers medication, develops, and implements nursing care plans, and maintains medical records.
  • May advise patients on health maintenance and disease prevention or provide case management.
  • Understands own work area professional concepts/standards, regulations, strategies, and operating standards.Required QualificationsUse your skills to make an impact
    • Current state licensure as an LPN or LVN in the state of Texas
    • 2+ years' experience as LPN or LVN in an inpatient or outpatient clinic setting
    • Experience with Electronic Medical Records.
    • Must be able to work at the Conviva Care clinic located at: 260 Tenth St. Floresville, TX 78114Preferred Qualifications
      • Hands-on professional Phlebotomy experience
      • Leadership experience
      • IV certification
      • Bilingual in English and Spanish, with the ability to speak to patients in both languages
      • Experience with HEDIS
