compassionate Case Manager to join our growing San Antonio team!

leader in quality home care across Texas. What You’ll Do: As a Case
Manager, you’ll play a vital role in coordinating care and
supporting both clients and caregivers. Your responsibilities will
include: Coordinating Starts of Care and Supervisory Visits
Communicating with State Program and MCO Case Workers Supervising
Personal Care Attendants Assisting with billing, scheduling, and
office operations Collaborating with HR to maintain up-to-date
staff files Providing direct personal care when needed If you have
experience with STARPLUS, STAR Kids, PHC, MDCP , or similar
programs, we want to hear from you! What We’re Looking For: High
school diploma or GED (required) 2 years of supervisory experience
Experience managing PAS programs (preferred) Strong communication
skills (Spanish-speaking a plus!) Comfortable with MS Word, Excel,
and basic office equipment A heart for service and a commitment to
