Our team supports not only our patients, but also their families, ensuring dignity, comfort, and peace during every stage of hospice care. Position Summary We are seeking a PRN Master Social Worker (MSW) to join our hospice care team. The ideal candidate will provide emotional, psychosocial, and resource-based support to patients and families, helping them navigate end-of-life care with dignity and understanding. Key Responsibilities Assess patients’ and families’ psychosocial, emotional, and support needs. Develop and implement individualized care plans in collaboration with the interdisciplinary team. Provide counseling, crisis intervention, and grief/bereavement support. Assist families in accessing community resources and support services. Maintain accurate and timely documentation in accordance with state, federal, and agency guidelines. Participate in care conferences, team meetings, and patient/family education. Qualifications Master’s degree in Social Work (MSW) from an accredited program. Current Texas licensure as LMSW. At least one year of hospice or palliative care experience preferred. Strong communication, counseling, and advocacy skills. Ability to work independently and collaboratively in a team setting.
